Independent Patient Policy

At Camberley Dental Clinic, we are committed to delivering high-quality dental care in a professional, ethical, and patient-focused environment. In order to ensure appropriate and fair access to our Independent dental services, prospective Independent patients must meet the following eligibility criteria:

• Be 18 years of age or older.
• Not be currently registered with another general dental practitioner.
• Not be undergoing active dental treatment with another dental practice at the time of application.
• Not currently have a dental emergency*, pain* nor any other dental issue*.
• Agree to attend regular dental check-ups and hygiene appointments as recommended by the dentist.
• Have the capacity to provide informed consent for dental treatment.
• Disclose any ongoing medical conditions, especially those that may affect dental treatment (e.g., bleeding disorders, diabetes, pregnancy, immunocompromised conditions).
• Disclose a complete and honest dental history, including past treatments, dental phobias, etc.
• Inform the practice of any allergies, particularly to dental materials (e.g., latex, antibiotics, local anaesthetics).
• Adhere to appointment schedules and provide at least 48 hours’ notice(not including weekends and bank holidays) for cancellations or rescheduling. Failure will result in a payable fee.
• Understand that Independent dental care covers only routine and basic treatments*.
• Agree to sign and acknowledge treatment plans incl. fees*.
• Agree to be seen Mondays – Fridays between 10:00am and up to 16:00 pm.
• Treat all members of staff and other patients with courtesy and respect.
• Abide by the practice’s payment policy, including prompt payment for all appointments and treatments.
• Understand that failure to comply with these expectations may result in withdrawal from active patient’s list.

Registration and Appointments:

• Initial consultation fee: £27.40, payable in advance and non-refundable.
• Follow-up appointments and treatments will be charged separately based on the agreed treatment plan. A deposit may be taken to secure your next appointment.
• A 48hour’s notice cancellation( not including weekends and bank holiday), is required to cancel or rearrange appointments. Your deposit will be forfeit if notice is not given or if you arrive more than 10 minutes late for appointment.

Payment:

• All fees are payable prior the appointments, a deposit may be taken for follow-up treatment and routine examinations.
• All major debit and credit card accepted (except Amex).

Treatment Planning:

• Agree to undergo a private new patient examination, which may include radiographs and photographs, before any treatment is planned or undertaken.
• Be prepared to sign an independent Patient Agreement, including consent to the treatment plan and acknowledgment of fees.
• Be prepared to secure further treatment appointments and routine examinations with a deposit.
• Ensure to disclose any relevant changes to your medical history and personal details for accuracy of paperwork and treatment.

Emergencies:

• We do not provide Emergency care for Independent Patients.

Privacy and Data Protection: 

• Patient information is collected, stored, and used in accordance with GDPR and UK data protection laws.
• Details of how your information is used can be found in our Privacy Notice.

Referral Policy:

• If you need to be referred for treatment, we can only refer you on a private basis to other providers.

Discretionary Acceptance:

• Acceptance as an Independent patient is at the sole discretion of the practice management.
• The practice reserves the right to decline an application without obligation to provide a reason if it is believed that acceptance would not be in the best interests of the practice or the Independent patient.

 

 

Emergency, Pain or any other Dental Issue:

• Should this be found at your first appointment, you’ll be charged a Private Emergency Fee £80 and this appointment will no longer be on the basis of Independent patient.
• Independent dental care covers only routine and basic treatments: should you require treatment outside the scope of Independent care, prices will be that of our Private list fees.
• Fees are reviewed annually.​​
